是否可以理解为插入语?改为:

North Korea said for the first time that a Covid-19 outbreak had rapidly spread throughout the country.

有的传统语法书认为it said for the first time是插入语。但这种分析其实是欠妥的。现代语法认为这种定语从句为嵌入式定语从句,即定语从句本身是个主从复合句,关系代词在其中的that从句中做成分。由于that从句是定语从句中的嵌入分句,所以把这种定语从句称为嵌入式定语从句(其实是嵌入式关系分句)。你可以在本网搜索阅读嵌入式定语从句。
